Tax & self-assessment
Personal and partnership returns covering NHS, private and locum income, with every relief claimed and the figures explained in plain English.
We act only for doctors, dentists and the practices they build. One named accountant, a fixed monthly fee, and advice through the year — not a single message at tax time.
A taper, worked
£40,000
The allowance that disappears for a consultant on £340,000 adjusted income. We run the calculation in spring, while there is still time to act.

Who we help
The levers are different for a salaried GP, a locum and a partner with three associates. Pick the closest description and see what we actually watch for you.
GP partner
Partnership accounts, drawings projections and the superannuation certificates that decide your pension position. We model next year's drawings before you commit to them, and flag the year a buy-in or buy-out changes everyone's tax.
Partnership accounts and each partner's allocation
Drawings projected against your pension allowance
Type 1 and Type 2 certificates issued on time
Buy-in, buy-out and the retirement of a partner
How it's usually engaged: Monthly retainer for the partnership, plus each partner's own return
What's included
Specialist advice, connected across your practice, pension and personal tax.
Personal and partnership returns covering NHS, private and locum income, with every relief claimed and the figures explained in plain English.
Type 1 and Type 2 certificates, allowance and taper modelling, Scheme Pays and flexi-access, and a retirement plan built on your scheme rather than around it.
Status reviewed for each engagement, umbrella versus your own company compared on real numbers, and contracts read before you sign them.
Partnership, limited company and PCN accounts prepared on a calendar you set, with reporting that a clinical lead can actually use.
PAYE for practice staff, pension auto-enrolment, and a Xero ledger that stays reconciled month to month instead of at year-end.
Incorporation and private practice set-up, buy-in and buy-out, practice sale, and an exit that works with the pension you've spent a career building.
NHS pension spotlight
Once threshold income passes £200,000 and adjusted income passes £260,000, your allowance falls by £1 for every £2 of adjusted income above that line, until it reaches a £10,000 floor. The number that eats it is the capitalised growth on your NHSBSA statement — not the contributions taken from your payslip.
We model that figure from your statement in spring, weigh Scheme Pays against flexi-access and carry forward from the previous three tax years, and tell you before the statement lands rather than after.
£60,000
Standard annual allowance for the 2026/27 tax year
£200,000
Threshold income above which the taper can begin
£260,000
Adjusted income above which your allowance starts falling
£10,000
The floor, reached once adjusted income hits £360,000
Figures are the annual allowance and taper thresholds for the 2026/27 tax year and are shown for general information. They are not personal tax advice, and they move with the tax year.
Why a specialist
A generalist practice
You hear from them once a year, around the deadline
The NHS pension is treated like any other pension pot
IR35 is a question nobody has put to them before
Fees arrive as an hourly invoice you can't predict
Medical.Accountants
A named accountant, reachable all year, who replies within one working day
Allowance and taper modelled from your statement before the year closes
Status reviewed engagement by engagement, and written down
One fixed monthly fee, agreed before we start
Switching
You talk, we ask. No slides and no obligation to move — you leave with at least one thing to fix wherever you are.
The moment you say yes, we write for the ledger, the files and the pension correspondence. Chasing is our job, not yours.
One monthly figure, what sits inside it, and what would cost extra. If your situation changes, the fee changes by agreement.
We rebuild your position — income, pension, structure — and put next year's numbers in front of you before the tax year is done.
Packages for your working life
Self-employed income, a limited company, PAYE or a mixture: start with the support that matches how you actually work. We check your status and agree the scope and fee before starting.
For genuine sole-trader income. We prepare your business accounts and Self Assessment, review allowable expenses and explain payments on account.
Fixed fee quoted after a review
Discuss this packageFor clinicians working through their own company. Choose Essential/Compliance, Core/Complete or Growth according to your accounts, bookkeeping and payroll needs.
Company packages from £45.83 + VAT / month
Compare company packagesFor employed clinicians, partners and people with mixed income who need a return. We check whether Self Assessment is required and reconcile the relevant income, expenses and pension tax information.
Fixed fee quoted after a review
Discuss this packageEmployment status is determined by the facts, not the package you choose. PAYE, umbrella and partnership arrangements may also apply. A limited company does not override IR35 or off-payroll rules, and not every employed clinician needs a tax return.
Overpaid tax?
We review tax codes, eligible professional expenses and overpaid tax, then prepare a claim where you qualify. A refund is not guaranteed; HMRC decides the claim.
£249 + VAT
For refunds up to £5,000, our fee is £249 plus VAT. For refunds above £5,000, add 5% of only the amount above £5,000 to the £249 fee. VAT applies to the total professional fee.
Example: a £6,000 refund means £249 + £50 = £299 plus VAT.
Professional fee, excluding VAT
£249
This illustrates the fee only, not whether you qualify or how much HMRC will refund. We agree the claim scope and payment terms before work starts.

What people arrive saying
These are the sentences medical professionals actually open with. They aren't client quotes, and none of them are unusual.
Pension
“My pension savings statement arrived and the input amount looks enormous.”
The taper
“I went over £200,000 last year and nobody has explained what the taper actually does to me.”
Locum status
“I work through an agency and directly for two practices, and I couldn't tell you which is inside IR35.”
Structure
“Everyone says I should incorporate. Nobody has shown me the numbers.”

One named accountant
No account manager relay, no rotating pool of juniors, and no starting the story again every time you need something. One person holds the whole picture — your pension, your practice and your personal return — and it's the same person the year after next.
Questions
Yes. Every client is a doctor, dentist, pharmacist, optician or allied health professional, or a practice, partnership or PCN. That focus is why we know the pension certificates and the IR35 questions a general practice rarely sees.
Yes. Plenty of people arrive long after the autumn deadline. We pick up where your previous accountant left off and the fee is pro-rated to the work that's actually left to do.
We write to them for the ledger, the files and the correspondence the moment you confirm. It's a handover we handle constantly — you shouldn't have to have the awkward conversation.
We check Type 1 and Type 2 certificates against what you've actually earned, model the allowance and the taper, and tell you before the statement lands whether you're heading for a charge.
One fixed monthly figure, agreed in writing before you commit. If your circumstances change materially, we agree a new figure rather than quietly adding to the old one.
Within one working day, from the person who will actually handle your work. If it's urgent, say so and we'll tell you honestly when we can get to it.
